- "Shopper" is defined as an individual providing shopping services. "Client" is defined as an individual receiving shopping services. I authorize the Portland Food Co-op to share my name, email, phone number, and location with other individuals registered in the Substitute Shopper Program. I understand that my information will not be shared, sold, or used for any other purpose outside of the Substitute Shopper Program. I understand that my participation in the Substitute Shopper Program is voluntary and I can request to be removed at any time by emailing marketing@portlandfood.coop and requesting to be removed. I understand that I am not guaranteed any service by filling out this form, and that the Co-op's ability to match participants is dependent on the availability of matches, staff availability, and other unforeseeable factors. The Co-op will do its best to match shoppers and clients within 72 hours, but this may not be possible for all who register. As a shopper: I understand that this is an opt-in program that I am willingly volunteering for, and that registering to be a Substitute Shopper does not make me an employee, contractor, or representative of the Portland Food Co-op. I will not seek remuneration of any kind from the Co-op. If an issue arises between myself and my matched client, I will not involve the Co-op in any way. I am not entitled to payment, tips, or reimbursement other than the cost of the groceries (and Casco Bay Lines freight costs, if applicable) I have purchased for my client, which is my client's responsibility to pay for. As a client: I understand that the shopper I am matched with is a volunteer and is not an employee, contractor, or representative of the Portland Food Co-op. If an issue arises between myself and my matched shopper, I will not involve the Co-op in any way. I agree to pay my shopper for the groceries (and Casco Bay Lines freight costs, if applicable) that are purchased on my behalf by my shopper. Please note that clients and shoppers should discuss the best way to coordinate payment/reimbursement with one another. Since every situation is different, the Co-op leaves the coordination of payment/reimbursement up to clients and shoppers to determine the best method to accommodate both parties. Please also note that the Co-op's point of sale system is unfortunately unable to process advance payments on purchases or accounts. Liability Notice: Portland Food Co-op is not liable for any injury or damage to personal items as a result of volunteering for or receiving services from this program. Portland Food Co-op is not liable for or will be involved in disputes if any kind between matched individuals. I understand that after I receive information regarding my match that all further communication and organization of services is the responsibility of both myself and my matched individual and that the Co-op will not mediate or organize further communication or services. -
